Topiramate in Rett syndrome: brands, hydration and side effects

How it may be used
Topiramate is a broad-spectrum antiseizure medicine and was among the five most commonly reported medicines in a large international Rett cohort. It may be selected for several seizure types, but individual response and tolerability vary.

Common side effects
• Tingling in the hands or feet
• Tiredness, sleepiness or dizziness
• Reduced appetite and weight loss
• Feeling sick, diarrhoea or stomach discomfort
• Tremor
• Slower thinking, attention, word-finding or communication
• Low mood or other behaviour changes

Important risks and monitoring
• Reduced sweating and overheating, especially in children or hot weather
• Kidney stones
• Metabolic acidosis
• Sudden eye pain or blurred vision from acute glaucoma
• Increased ammonia or encephalopathy, particularly with valproate
• Effects on growth, nutrition and bone health
• Significant pregnancy risks requiring specialist review and pregnancy-prevention measures where applicable

Maintaining appropriate hydration is important unless the clinical team has advised fluid restriction.

Seek urgent medical advice for
Eye pain or sudden vision change, inability to sweat with fever or overheating, severe weakness, fast breathing, unusual confusion or sleepiness, severe side/back pain, blood in urine, a severe rash or major mood change.

Important: Never stop topiramate suddenly.
