Key points to discuss:
- Include diagnosis, baseline breathing and movement patterns, seizure plan, medicines, allergies, swallowing status and feeding method.
- Explain the person’s usual communication, pain signs, calming strategies and how they show yes/no.
- List mobility, positioning, pressure-care and equipment needs.
- Provide emergency contacts and key specialists, plus copies of recent ECG and important letters where relevant.
- Ask for reasonable adjustments, carer involvement and a clear discharge plan.
